About the Role:
CenClear's Behavioral Health Division is seeking a compassionate Licensed Practical Nurse (LPN) to support individuals in both community and clinic settings. This unique role offers a 50/50 split between community-based and clinic-based work, providing variety and meaningful impact.
This position requires professionalism, strong attention to detail, and a commitment to confidentiality in a behavioral health environment.

What You'll Do:
- Schedule:
40 hours per week, 52 weeks per year
- Community-Based Responsibilities (50%)
- Provide in-home medication management services to individuals with mental health, physical health, or intellectual disabilities
- Review, verify, and educate individuals on prescribed medications, including purpose, administration, and side effects
- Support medication adherence and promote independence in self-management
- Monitor compliance through in-person visits and phone follow-ups
- Maintain accurate documentation in the electronic health record (EHR)
- Communicate concerns and collaborate with case managers and care teams
- Coordinate care with healthcare providers, pharmacies, and community resources
- Identify additional needs and facilitate referrals to appropriate services
- Travel to consumer homes and maintain confidentiality and professional standards
- Clinic-Based Responsibilities (50%)
- Provide primary coverage at the St. Marys office, with occasional support at DuBois and Clearfield locations
- Assist psychiatric prescribers by collecting vitals, labs, reports, and relevant client information
- Update client histories and follow up on care as directed
- Deliver wellness coaching and promote health education to clients, staff, and community partners
- Collaborate with providers to ensure continuity of care and facilitate referrals
- Collect and document data for outcomes tracking and program requirements
- Complete documentation, reports, and administrative tasks accurately and on time
- Participate in required trainings, meetings, and monthly supervision
What We're Looking For:
- Current Pennsylvania Licensed Practical Nurse (LPN) license
- Minimum of two years of mental health experience preferred
- Strong clinical judgment and ability to work independently
- Excellent communication and interpersonal skills
- Strong organizational skills with attention to detail
- Commitment to confidentiality and professional standards
- Ability to manage multiple priorities effectively
- Proficiency with computer systems and Microsoft Office preferred
- Valid driver's license, reliable transportation, and willingness to travel
- Flexibility to work occasional evenings based on client needs
- Must obtain the following clearances prior to hire: PA State Police Criminal History, PA Child Abuse History, FBI Criminal History, and Mandated Reporting Certificate
